For your first visit you will need to read and fill out the forms (below).  This is about 30 minutes of work.  Please prepare these ahead of time, and email to me so that I may review them prior to your first visit. (Please note- no need to fill in your SS#, since you likely will email these forms to me).

How to Request Your EarthSong Records:

You may request copies of EarthSong records for yourself, or to be transferred to the clinic of your choice using this form:

You may return this to me via email (as noted on the form), postal mail, or fax. I will respond within one week.

Records Request Fees: All records requests will be processed once weekly, and may incur a $25 fee (depending on the size of your chart, and method selected for transfer of records).
